New Patient Center
We try to keep paperwork simple and expedient. On your first visit there are a few pages to fill out for our records, if you need help with the writing please ask Kim at the front desk for assistance. Your appointment time is planned to provide plenty of time for the paperwork so you don't need to arrive before your scheduled time.
Please do be on time or call and let us know if you are running late. We make every effort to book our schedule to run smoothly and see each patient at their appointed time, therefore we do NOT over book. This means if you are late then we are late for the next patient or we must put them in ahead of you which changes when you will be finished and out of the office.
We ask everyone to give us at least 24 hours notice if you need to change your appointment time. We realize this is not always possible but do call as soon as you know you will be unable to make your appointment on time.
We reserve the right to charge for appointments missed and not cancelled ahead.
We do have phone service 24/7 so please call anytime. If it is after hours and I don't pick up the phone leave a detailed message.
